If you wish you can fill in a registration form before you attend the surgery. We would ask you to bring in 2 forms of ID to register if you have them – one photo ID and one confirmation of address. When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Please be aware that if not all areas of the form are completed we will not be able to register you.
