To register at Norwood we can only accept patients who live in our practice area. The reception staff will be able to tell you if you live within the area and also how to register.

If you cannot complete a registration form online, you can collect a form from reception.
Medical treatment is available from the date of registration and not the date you hand your forms in. Please contact reception for further information.
